What is it?

  • a provider visit with management of diabetic meds., labs, and referrals
  • interactive diabetes curriculum including lifestyle management. Patients learn a lot from each other.
  • a major source of support: breaks isolation and builds community
  • meets every 2 weeks for 6 visits

    • your referral goes to a Workqueue, and the patient will receive a phone call to schedule the group appointments.
